Common causes for errors and faults of Raid systems:

Defects of hard disks which may appear only during rebuild or as direct consequence of it

Damaged or faulty controllers

Inadequate support by manufacturers, eg when advised to rebuild the Raid5, delete or recreate it for lack of other options

Inadequate user documentation in error cases

User errors (disks are exchanges intentionally; more than one disks is replaces; all available options in the controller bios applied)
